Telephone Service

ORDER TELEPHONE SERVICE

To receive telephone service through Plains Cooperative Telephone Association is easy. You are welcome to stop by our cooperative office located at 6488 U.S. Hwy 36, Joes, Colorado or call us at 970.358.4211. You will be asked to supply the following information:

* Your name or the name of your business

* Your Social Security Number

* Your Service Address and Mailing Address

* Previous Address and telephone number

* Employment Information

* How you would like your name to appear in our directory or if you prefer an unlisted number

* Long Distance carrier selection (a list will be provided to you)

* Which calling features you would like on your line

* Do you want Inside Wire Maintenance

Payment is required at the time you order your service.

Residential Rates Include: Service Order Charge $20.00, Central Office Connection $20.00 and First months local service $25.00. A Total of $65.00 must be paid at the time application is submitted for service.

For Long Distance Service you must provide one of the following: 1) good credit check - with your permission we will check credit through the Experian Credit Bureau. 2) Co-signer - must be a Plains Telephone customer with a good credit standing. 3) Deposit - $235.00 returned to you after credit has been established.

Business Rates Include: Service Order Charge $20.00, Central Office Connection $20.00 and First month's local service $30.00. A total of $70.00 must be paid at the time application is submitted for service. We also follow the same rules for long distance service as mentioned above for residential customers.

CALLING FEATURES AVAILABLE

Voice Mail $4.50/month - Never miss a phone call again. You can
access your voice mail messages from home or away.

Call Waiting $1.50/month - When you are on the telephone, a special tone alerts you that someone is calling.

Cancel Call Waiting $.50/month - Allows you to turn off Call Waiting before making an important call.

Caller I.D. (Name & Number) $4.25/month - See who is calling before you answer the phone. Name and Number will be stored on a Caller ID unit even if callers don't leave a message.

Caller I.D./CW Identity $1.00/month - See who the second caller is on your Caller I.D. unit before answering the call.

Automatic Callback (*66) $2.00/month - Enables you to automatically reach busy numbers. You may also use to redial the last number dialed from your phone.

Automatic Recall (*69) $2.00/month - Returns calls to the last number that called you, whether or not you answered the phone.

Call Forwarding (*72) $1.50/month - Forward your incoming calls to another number.

Remote Call Forwarding $.50/month - Subscribers may now turn their Call Forwarding feature "on or off" from any touchtone telephone. Must Subscribe to the Call Forwarding Feature before remote call forwarding will work.

Call Forwarding, Busy $1.00/month

Speed Dialing (8 digit) $1.50/month - Call emergency, frequently dialed, or long distance numbers just by dialing one or two numbers.

Speed Dialing (30 digit) $3.50/month - Make calling fast and easy by storing up to 30 frequently called numbers.

Three-Way Calling $1.50/month - Add a third party to a local or long distance conversation.

Help Line $1.50/month - Automatically dials a programmed telephone number within 30 seconds when receiver is lifted.

Distinctive Ring $3.00/month - Gives the subscriber a second telephone number which will ring differently on your phone.

Conference Bridging 358-001 (9 ports available) $1.00/port - A very affordable way to conduct business with several people at one time.

Additional Directory Listings $2.50/month

Unlisted Number $1.50/month - not in the directory but is available through directory assistance.

Unpublished Number $2.00/month - not in the directory or directory assistance.

Off premise Extension $2.50/month - if under 1/4 mile.

Pre-Recorded Messages $10.50/month - Great option if you are moving and want to inform callers of your new phone number.
